How do I get a copy of my (or my loved one's) birth, marriage, or death certificate?
You will need to check with the Burke County Office of the Register of Deeds. Their address is: 201 South Green Street, PO Box 219, Morganton, NC 28680-0219. How do I get a copy of my medical records?
If YOU are the patient:
You will need to complete an electronic trading platform in the UAE Authorization for Release of Health Information form. To have this form mailed or faxed to you, please contact the Health Information Management (HIM) Department, Mondays through Fridays from 7:30am until 5:00pm, via telephone: at Grace (828-580-6887) or at Valdese (828-874-2251 x380). You are also welcome to request an Authorization form by emailing the Director of Health Information Management at akane@blueridgehealth.org or by coming in person to the Hospital's HIM Dept, Mondays through Fridays from 7:30am until 5:00pm, to complete the authorization form. If you come in person, please be sure to bring a photo ID (such as a NC Drivers License) with you for identity verification. Please do NOT send another person in your place to complete the authorization form as he/she will not be able to do so by law. However, he/she is welcome to pick up a blank form for you to complete at your leisure. Your medical record copies will be ready for mailing or self pick-up in 2-4 business days. If you need to send another person in your place to pick up your medical record copies, he/she will have to bring a signed and dated letter from you which authorizes them to do this for you. Fulfilling medical record requests for the purpose of continuation of medical care is our top priority !

If you are not the patient but are requesting patient records for a family member there are several federal and state laws as well as other legal guidelines which will need to be followed: Please contact the HIM Dept., Mondays through Fridays from 7:30am until 5:00pm, via telephone: at Grace (828-580-6887) or at Valdese (828-874-2251 x380) for information on how to obtain a copy of the records.
Minors -- parents or guardians may sign for minors unless the record contains information that is protected under law: psychiatric, drug or alcohol abuse records. These records may only be released by the patient's signature or through an order from a court.
Deceased patients -- the executor of the estate is the only one who can authorize release of the patient's medical records after death. Once you have obtained the legal documents validating you as the executor of the estate, you can complete an Authorization for Release of Health Information form and the copies will be provided to you within 5 business days.

PLEASE BE AWARE that North Carolina law specifies medical record copy fees at all NC hospitals.
